What is longevity medicine?

Longevity medicine is a proactive approach to health that uses data, biomarker testing, and evidence-based protocols and interventions to support healthy aging. What medications does Bryan Johnson take?

Bryan Johnson takes a range of prescription medications to support longevity, including Acarbose, Daily Tadalafil (Generic for Cialis®), Minoxidil (both oral and topical), Metformin (cycling), Tretinoin, Jardiance®, Candesartan and Repatha®. These are all FDA-approved medications, often prescribed off-label for longevity support.

Why is tadalafil prescribed for longevity?

Tadalafil (Generic for Cialis®) is often prescribed as an FDA-approved medication to treat specific conditions, including erectile dysfunction, but it’s an emerging treatment prescribed off-label by some providers within a longevity medicine framework.

The same mechanism that causes ED (blood vessels losing their ability to relax and open) is often an early stage of cardiovascular disease. 

Because Tadalafil works by improving how blood vessels function throughout the entire body — not just in the penis — researchers started looking at whether it might also protect the heart and brain. The data so far is striking: large observational studies show men who take PDE5 inhibitors like Tadalafil have significantly lower rates of heart attacks, heart failure, stroke, and even Alzheimer's disease. While the research in females is less abundant, the mechanism of action should, theoretically have the same effect on female cardiovascular health.

Can women take tadalafil for longevity?

The vascular mechanism that makes tadalafil relevant for longevity applies to women too.

Nitric oxide signaling and PDE5 activity govern blood vessel function in everyone — not exclusively in men. Tadalafil is already FDA-approved for pulmonary arterial hypertension, a condition that affects women at higher rates than men. That approval is based on demonstrated vascular benefit in female patients.

The cardiovascular longevity data (heart failure, stroke, and Alzheimer's associations) comes primarily from observational cohorts of men with ED, the population where PDE5 inhibitors have been used at scale. The endothelial mechanism doesn't change by sex. Research into tadalafil and female cardiovascular outcomes is ongoing.

Why is metformin prescribed for longevity?

Metformin is an FDA-approved medication for type 2 diabetes that has been extensively researched for its role in healthy aging. It is commonly used off-label within longevity protocols to support metabolic health. What is compounded or personalized medication?

Compounded medications are customized prescriptions prepared by licensed compounding pharmacies. These are tailored to a patient’s specific clinical needs, such as personalizing a dose based on health goals for which commercially available doses may not be appropriate. A compounded medication may also combine multiple active ingredients to suit a patient's unique needs. While these medications use doctor-trusted ingredients from licensed pharmacies, the compounded formularies are not FDA-approved.

Who is eligible for Immortals Medicine?

Immortals Medicine is available to adults 18+ in eligible U.S. states. Certain medications are not available in certain states.

Eligibility for any medication is determined solely by a licensed healthcare provider based on your medical history, biomarkers, and an individual clinical evaluation. The 5-10 minute intake form is the first step to determine which treatments you may be eligible for. A physician reviews every case before any prescription is issued.

How much does Immortals Medicine cost?

Pricing varies by medication and dose. Immortals Medicine has no membership fees or insurance requirements. You are only charged after a physician approves your treatment. 

The intake takes about 10 minutes, with doctor review typically completed within 24 hours. Once reviewed and if prescribed, most medications ship in 2–5 business days. The cost of your subscription includes ongoing, unlimited messaging support with your healthcare provider regarding your protocol for 12 months. 

Does Immortals offer longevity medications for women?

Yes. Immortals Medicine offers female-specific longevity medications including estradiol and tadalafil for longevity, prescribed by licensed physicians.

Most longevity medicine wasn't built for women. The large outcome trials, the protocol defaults, and the dosing assumptions were developed in male cohorts, applied to women as an afterthought. The underrepresentation of women in longevity research is a major gap. It's why most longevity platforms default to a women's version of a male protocol.
